Students may complete background screening application online (only ONE FORM this way versus two the traditional way) and return to Education Department for final processing.

1. Students can complete the form on the computer and then print it off (recommended)*.

2. A COPY of the student's social security card will be required.

3. Checks should be payable to MVC/S-MSTA.

Return complete package to Edcuation Department for final processing.
